Directions to our Fort Worth office:

 

If you are coming South on Bryant Irvin Road:
Turn left on Oakmont Boulevard. Go through the first light (Harris Parkway). Once through the light on the left hand side of the road you will see a Hawkins Business Park sign. You will then see the street Hawkins View Drive, turn left. Once on Hawkins View Drive, take your first left and then at the stop sign, turn right. We will be the 2nd parking lot on the right. Our door faces North, you will see it when you have turned into our parking lot.

If you are coming South on Hulen Street:
Turn right onto Oakmont Boulevard. You will pass Baylor Southwest Hospital on your right. A little past the hospital, you will see a sign for Hawkins Business Park. You will see a street called Hawkins View Drive, turn right onto this street. Once on Hawkins View Drive, take your first left and then at the stop sign, turn right. We will be the 2nd parking lot on the right. Our door faces North, you will see it when you have turned into our parking lot.

Appointments & Cancellations

To make an appointment, please call 817-529-6200.

We are aware of and sensitive to the fact that an emergency may occur in a rare instance, preventing you from keeping your scheduled appointment. However, cancellations, especially last minute ones, along with patient no-shows, limit our ability to accommodate the scheduling needs of other patients. We therefore ask for your full cooperation with the following policy. Failure to cancel or reschedule an appointment with 24 hour notice will result in a $50 cancellation fee.

This policy is necessary for the benefit of all our patients, so that we may continue to provide high quality treatment and service to everyone. We appreciate your anticipated adherence and cooperation with this policy.
